The Church Health Survey^® is a 160-question diagnostic assessment tool developed by Dr. Thom Rainer of Church Central Associates (www.churchcentral.com). The survey is taken by 15% of the adult population of the church, or 30 people, whichever is greater.

The Church Health Survey^® does exactly what the name implies… it surveys the health of churches in six main areas of ministry as defined by Acts 2: Prayer, Evangelism, Discipleship, Fellowship, Ministry (serving), and Corporate Worship. 

Survey answers are completed on scantron computer sheets, and sent directly to Church Central Associates in Louisville, Kentucky. The surveys are graded, and the results are sent directly back to the pastor and church for their analysis.

All Church Health Surveys^® are routed through a licensed church consultant from Church Central Associates.

The Church Health Survey^® gives church leaders an immediate pulse on the strengths and weaknesses of their church.
